Forms and Brochures:
Forms Note from
the webmaster: Many people have written me to tell me that they
are experiencing problems with downloading PDF files from the
web site. Adobe Reader will tell them that the file is damaged.
I can not reproduce this problem from any of my machines, and
every time I check the files they are fine. Our web hosting
service can not explain the problem either. At least some of
the time, when people just retry the download, it works fine.
If and when I can pin down this problem, I apologize for any
inconvenience. Meanwhile, if you have a problem like this, please
tell me as much as you can about your browser, your connection
speed, your version of Reader or Acrobat, and anything else
that might help the support staff figure out why this issue
has arisen. Thank you.
This form may be completed on-line, printed from your computer and then faxed or mailed to the AGS office.
We hope this will prove to be a convenient choice for our members and eliminate some errors due to
handwriting interpretations. Soon we hope to offer actual on-line email submission directly to the AGS office.
After completing and printing the form, you may hit the "back" button on your browser bar to return
to this page, or click "Reset" to complete another form.
Of course, if you prefer to fill out forms by hand that form is still available below.
The Paypal account for AGS is: payags@earthlink.net
